Moderate to severe scalp psoriasis Clinical criteria: The condition must be inadequately controlled with either a vitamin D analogue or potent topical corticosteroid as monotherapy; OR The condition must be inadequately controlled with combination use of a vitamin D analogue and potent topical corticosteroid. Population criteria: Patient must be at least 18 years of age.
Corticosteroid-responsive dermatoses Clinical criteria: The condition must be resistant to lower potency topical corticosteroid therapy.
